Based on observations made during tour, facility policy review and staff interview, the facility failed to ensure two physicians followed the current infection control policy related to hand hygiene. This had the potential to affect all patients receiving care in the facility.
Findings include:
The facility's Cincinnati campus was toured on 07/07/20 at approximately 2:30 PM. A staff physician was noted to be providing care to a patient inside room #1564. Although the physician was noted to be wearing a mask, he/she was not wearing gloves while providing care which included touching the patient. The physician was then observed leaving the room without performing any hand hygiene. The physician began typing into a computer on wheels. These observations were confirmed with Staff A who was present during the tour.
Observation of a sign on the door of room #1571 revealed contact precautions were needed. A physician was observed entering the room. The physician did not perform hand hygiene prior to entering the room and interacting with the patient. This observation was also confirmed with Staff A during the tour.
The facility policy titled "Standard Precautions", revised 08/2019, was reviewed on 07/09/20 at 12:30 PM. According to the policy, staff are instructed that hand hygiene is required before and after every patient interaction even when gloves are used. These findings were confirmed with Staff A on 07/07/20 at 3:00 PM.
