Based on observations and staff interviews, the hospital staff failed to provide a clean and sanitary environment..

The findings include:

Observations of the ICU (Intensive Care Unit) conducted on February 6, 2013 at 0900 revealed the area in front of the nursing station desk had black line markings on the floor (appropriately 48 inches in lenght). Further observation revealed the entire floor at the nursing station sitting area and the physician dictation area had visible black scuff marks on the floor.

Observations during tour of the 3rd floor conducted on February 6, 2013 at 0915 revealed the equipment cart (in the hallway between patients' rooms) had visible lint accumulation (build-up) and brown substance on the floor. Further observation revealed the entire floor area at the nursing station sitting had visible black scuff marks.

Observations during tour of the 2nd floor conducted on February 6, 2013 at 0935 revealed the nutrition room with an accumulation of visible black particles covering the entire floor and water on the floor near the refridgerator. Further observation revealed food particles on the nutrition room counter top.

Ongoing observation of the 2nd floor revealed ceiling tile stains (brown in color) in room 210 (at entryway) and multiple black spots on the floor (at entryway). Further observation revealed the ceiling tile (in the hallway between patients' rooms) with brown-like stains.

Interview with hospital staff #3 conducted on February 6, 2013 at 1020 revealed, "...when a family complains about the floors, I let housekeeping know and they come and clean the room..."

Interview with hospital staff #4 conducted on February 6, 2013 at 1040 revealed, "these are old floors with cracks...we try to PM (Preventive Maintance: strip and wax floors)...but it's all related to census (patient)...EVS (Environmental Services Aide) is responsible or cleaning all patient's rooms, nutrition room and around the nursing desk..."

Review of the "SECOND FLOOR - FIRE WALLS & SMOKE BARRIER" revealed 13 out of 20 rooms did not received preventive maintance in 2012.
