Based upon observations & discussions, there are open penetrations in the smoke partitions at various locations throughout the facility which will allow the passage of smoke & flames from one smoke compartment to another.
Findings include that between the hours of 13:30 on 2 May and 15:45 on 5 May , accompanied by the Facilities Maintenance Director, the RRMC Fire Marshal and other maintenance staff items were noted: open penetrations in the smoke partitions throughout the facility in rooms and above the rated smoke door assemblies.

Based upon observations & discussions, structural steel structure in areas of the facility are not maintained to the required fire resistant ratings for the areas.
Findings include that between the hours of 13:30 on 2 May and 15:45 on 5 May, accompanied by the Facilities Maintenance Director, the RRMC Fire Marshal and other maintenance staff these items were noted: The steel structure in the main mechanical room, the main electrical room and at other locations throughout the facility had the required fireproofing material missing from the exposed steel structure of the facility.

Based on observations Exit Lights were inoperative.
Findings include that between the hours of 13:30 on 2 May and 15:45 on 5 May, accompanied by the Facilities Maintenance Director, the RRMC Fire Marshal and other maintenance staff these items were noted: The exit light from the gym to the pool corridor was inoperative. Upon this discovery, the maintenance staff corrected this deficiency before the survey was completed.

Based on observation and interview, the facility failed to provide electronic supervision of valves that are installed in an area that is not continuously occupied and failed to provide a fire alarm trouble signal in a location where it would likely be heard by staff.
Findings include that between the hours of 13:30 on 2 May and 15:45 on 5 May, accompanied by the Facilities Maintenance Director, the RRMC Fire Marshal and other maintenance staff these items were noted: The PIV at the entrance to the building was not connected to the Fire Alarm Control Panel. There was a switch at the PIV but it was not connected..

Actual NFPA Standard: NFPA 101, 9.6.1.4. A fire alarm system required for life safety shall be installed, tested, and maintained in accordance with the applicable requirements of NFPA 70, National Electrical Code, and NFPA 72, National Fire Alarm Code.

Based upon observations and discussions the Fire Sprinkler System Maintenance was incomplete.
Findings include that between the hours of 13:30 on 2 May and 15:45 on 5 May, accompanied by the Facilities Maintenance Director, the RRMC Fire Marshal and other maintenance staff these items were noted: there were corroded sprinkler heads in the kitchen near the dish washing machine areas.

Upon this discovery, the maintenance staff corrected this deficiency before the survey was completed
