Based on observation and interview, it was determined the facility failed to correctly position one sprinkler head, affecting two of two floors in this component.
Findings include:
1. Observation on October 22, 2014, at 11:17 AM, revealed the #1 exit stair tower did not have a sprinkler head under the lowest landing.
Exit interview with the Vice President of Facilities and maintenance representative #1 on October 22, 2014, between 1:00 PM and 1:15 PM, confirmed the sprinkler head at the bottom of the #1 exit stair tower was located five feet higher than the lowest landing.
