Based on review of records and interview the facility did not comply with 489.24 in that medical screening was not provided for Patient #20.
Findings:
1) In interviews with the surveyor the afternoon of 2/12/2010 the registration clerk and the Director of Emergency Services were asked if the patient was provided a medical screening exam. They confirmed she was not.

Based on review of records and interview the facility did not maintain an emergency log that included that the patient (Patient #20) presented to the facility.
Findings:
1) Review of the emergency department logs from August of 2009 through the date of the survey did not include the patients name.
2) In an interview with the surveyor the afternoon of 2/12/2010 the emergency department clerk was asked if the patient or her mother had presented to the facility for treatment. She stated they had.

Based on interview the hospital did not provide a medical screening examination for the patient (Patient #20).
Findings:
In an interview with the surveyor the afternoon of 2/12/2010 the emergency department clerk if the patient had presented at the emergency department. She stated the patient's mother came into the registration area, upset and requesting a wheelchair. As she obtained a wheelchair she asked the mother the nature of the patient's complaint. The mother stated the patient was drugged and had been raped. The clerk and the mother went to the parking lot, assisted the patient into the wheelchair and returned the registration area. At that time the Director of the Emergency Department spoke with the patient and her mother.
In an interview with the surveyor the afternoon of 2/12/2010 the Director of the Emergency Department was asked if she had spoken with the patient and her mother and if the patient had received a screening exam. She confirmed that the patient was not taken into the emergency department and did not receive a medical screening examination. The Director of the Emergency Department further stated that the hospital did not perform rape exams and she directed the patient and her mother to a another hospital 10 miles south.
