Based on observation, interview and record review the facility failed to maintain a clean and safe facility. There were debris that posed a fire hazard, standing water and mold. Peeling base boards, chipped paint and broken tile.
This failed practice had the potential to adversely affect all 16 patients on census.
Findings:
Review of complaint narrative (TX 00160773) revealed information that termite was seen in the facility including a patient's room and bed. There was standing water and dead trees on the facility's grounds.
During the investigation the following was revealed:
During an interview on 7/6/2012 at 9:00 am with the maintenance supervisor he stated there was a report of ants and termites in the facility and also in a patient's room. His investigation did not find any termite in the patient's room however there was termite at the door of the cafeteria and in an outside atrium.
According to the maintenance supervisor the pest control service came and sprayed the areas where the insects were seen. The were informed these were swarms that comes up periodically.
During a telephone to Pest Control staff Agent X who responded to the call he stated swarms form as a result of a nest growing too large and a swarm colony is formed else where. Agent X stated the termites usually borrow in wood and require more detailed treatment by individuals who specialized in treating termites. According to Agent X the treatment he did was a temporary fix. The situation could;d recur.
During an interview on 7/6/2012 with the facility's administrator he stated he was not aware that the treatment of the insect problem was a temporary fix.
Observation on 7/6/2012 at 9:45 am during a tour of the facility's grounds revealed thirteen (13) windows all leading form patient rooms to the back and side of the facility. The windows had wood panels that raised the windows approximately three feet off the ground. The wood panels were covered with heavy buildup of moist black mildew/mold looking material and the glass panes of the windows were dirty( dust web and dirt). Five (5) side windows had a buildup of dirt and slight mildew on the wood panel and the glass panes were also dirty.
There was standing water from an AC roof drain at the corner of the building near patient rooms. The waster soaked area had green morass and was very soggy.

Observation in the grounds to the rear of the building revealed five dried dead trees and five large mounds of dried branches and leaves. There was heavy buildup of dried debris along the periphery of the grounds. there were trees with dry hangings that needed trimming.
There was an outside patio to the back of the facility with several discarded items including beds, bedside tables, and pieces of board. All the identified debris were fire hazard.
Observation in the cafeteria revealed the base boards and floor were dirty. The steam table had dust and splatter in the corners and the covers of the steam trays. There was build up of dust on the equipment in the cafeteria and the hand wash sink was dirty and greasy.
Observation in the lobby revealed an area with broken floor tile. There were chipped paint and chipped side railings.
During an interview on 7/6/2012 at 1:30 pm with the Housekeeping supervisor he stated he would ensure the cafeteria was cleaned up.
Review of the facility's housekeeping activity report revealed instructions for daily cleaning in the cafeteria. There was no evidence that this process was followed.
