Based on visual observation the facility failed to provide smoke resistive walls and windows in the single front lobby.

Findings: During tour of building on 6-25-2010 at 12:00 p.m. the office areas located adjacent to the front lobby are not smoke resistive in the following areas 1. Doctors suite and intake office front glass sliding windows and 2. the marketing office glass door has no positive latching device

Based on visual observation and confirmed with the maintenance supervisor the facility failed to provide key overrides for 2 of the 5 exit locations.

Findings: During tour of the facility on 6-25-2010 at 11:30 a.m. the back exit door which has a magnetic lock and the gate outside of the adult wing that has a combination lock were observed not to have a key override enabling staff to unlock the doors quickly in the event of a emergency. This deficient practice could cause harm to the 24 patients house at the facility at the time of the survey.

Based on visual observation the facility failed to maintain the sprinkler system in accordance with NFPA 13 and Life Safety Code.

Findings: During tour of facility on June 25, 2010 at 10:30 a.m. and confirmed with the maintenance supervisor the following sprinkler deficiencies were observed 1. The back entrance porch which is over the four foot requirement to be protected and had a combustible ceiling did not have sprinkler coverage 2. There was no sprinkler protection in the treatment room closet 3. Suite A hallway did not have adequate sprinkler protection lacking 2 sprinkler heads 4. corridor on left side of adult nurses station and inside of this station does not appear to have enough sprinkler coverage 5. the sprinkler caps in the adult unit corridor have been painted and sealed to the ceiling, disabling them from providing water application in case of an emergency. This deficiency if not corrected could affect the 24 patients housed in the building at this time.

Based on visual observation the facility failed to provide ashtrays of safe design and self-closing covered containers to empty the ashtrays in 3 of the 3 areas where smoking is permitted.

Findings: During tour of building on 6-25 -2010 at 11:00 a.m. the following areas where smoking is allowed by patients and staff were observed not providing a ashtray of safe design and a metal container with a self closing cover into which ashtrays could be emptied 1. employees smoking area 2. Geri smoking area and 3. adult smoking area. This deficiency if not corrected could have the potential to cause harm to the 24 residents and staff
