Based on policy review, medical record review, and staff interview the hospital staff failed to provide a daily bath to a patient for 4 of 7 sampled patients (Patient #1, #6, #8 and #9).

The findings included:

Review on 12/05/2019 of the hospital policy titled, "Hygiene" revised 07/2018, revealed " ...Patients will be provided with hygienic care (specifically a bath) on the day shift seven days per week, and as needed ... The Primary Care Nurse is (overall) responsible for ensuring that patients are bathed and hygiene needs are met. ..."

1. Open medical record review of Pt (Patient) #6 on 12/04/2019, revealed Pt #6 was a 47-year-old female admitted on 11/15/2019 to the LTAC (long term acute care hospital) for Acute Hypoxic Respiratory Failure. Review revealed, Pt #6 required assistance with bathing. Record review revealed on 11/15/2019 at 1500 NP #1 ordered Pt #6 up ad lib (up out of bed as desired). Record review failed to reveal the patient received a bath on 11/16/2019, 11/17/2019, 11/24/2019, 11/25/2109, 11/28/2019, 12/02/2019 and 12/03/2019 in the bathing record.

Interview with CNO (Chief Nursing Officer) on 12/05/2019 at 1015, revealed patients are to be offered a bath daily. Interview revealed, when the patient received or declined the offer of a bath, it should be documented into the patient's medical record. Interview with CNO confirmed the observation of the bath record which failed to reveal the offer or decline of a patient's daily bath.

Interview with CNA (Certified Nurse Assistant) #1 on 12/05/2019 at 1130, revealed patients are to be offered a bath daily. When the patient received or declined the offer of a bath, it should be documented into the patient's medical record.

Interview with RN (Registered Nurse) #1 on 12/05/2019 at 1130, revealed patients are to be offered a bath daily. When the patient received or declined the offer of a bath, it should be documented into the patient's medical record.

Interview with Pt #6 on 12/05/2019 at 1230, revealed Pt #6 stated, "I was not offered a bath daily." Interview revealed, the patient required assistance with bathing.

2. Closed medical record review of Pt #8 on 12/04/2019, revealed Pt #8 was a 58-year-old male admitted 10/15/2019 through 11/27/2019 to the LTAC for wound care and ventilator weaning back to his tracheostomy (an artificial opening through the neck into the trachea, usually for the relief of difficulty in breathing) collar. Review revealed, Pt #8 required assistance with bathing. Record review failed to reveal the patient received a bath on 10/16/2019, 10/18/2019, 10/22/2019, 10/26/2019, 10/27/2019, 10/28/2019, 11/05/2019, 11/06/2019, 11/07/2019, 11/08/2019, 11/11/2019, 11/12/2019, 11/16/2019, 11/17/2019, 11/20/2019, 11/23/2019, 11/25/2019, and 11/26/2019 in the bathing record.

Interview with CNO (Chief Nursing Officer) on 12/05/2019 at 1015, revealed patients are to be offered a bath daily. Interview revealed, when the patient received or declined the offer of a bath, it should be documented into the patient's medical record.

Interview with CNA (Certified Nurse Assistant) #1 on 12/05/2019 at 1130, revealed patients are to be offered a bath daily. When the patient received or declined the offer of a bath, it should be documented into the patient's medical record.

Interview with RN (Registered Nurse) #1 on 12/05/2019 at 1130, revealed patients are to be offered a bath daily. When the patient received or declined the offer of a bath, it should be documented into the patient's medical record.

3. Closed medical record review of Pt #9 on 12/04/2019, revealed Pt #9 was a 47-year-old male admitted on 10/18/2019 through 11/06/2019 to the LTAC for antibiotic treatment for bacteremia (bacteria in the blood). Review revealed, Pt#9 required assistance with bathing. Review revealed on 10/17/2019 at 1500, NP #1 ordered Pt #9 up ad lib. Record review failed to reveal the patient receiving a bath on 10/18/2019, 10/21/2019, 10/23/2019, 10/25/2019, 10/27/2019, 10/28/2019, 11/01/2019, 11/02/2019, 11/03/2019, 11/05/2019, and 11/06/2019 in the bathing record.

Interview with CNO (Chief Nursing Officer) on 12/05/2019 at 1015, patients are to be offered a bath daily. Interview revealed, when the patient received or declined the offer of a bath, it should be documented into the patient's medical record.

4. Open medical record review of Pt #1 on 12/04/2019, revealed Pt #1 was a 68-year-old male admitted on 11/16/2019 to the LTAC for Acute Respiratory failure. Review revealed, Pt #1 required assistance with bathing. Record review reviewed on 11/16/2019 at 1230, NP (Nurse Practitioner) #1 ordered Pt #1 for up ad lib.
Record review failed to reveal the patient receivied a bath on 11/24/2019, 11/26/2019, 11/27/2019 and 12/3/2019 in the bathing record.
