Based on visual observation the facility failed to assure that hazardous areas are separated from all other use areas including the egress corridors by appropriately partitions that would resist the passage of smoke and fire and meet the required fire rating and potentially affecting 19 of the 19 residents of the facility.
Findings:
During facility tour on 05/07/2010 between the hours of 8:30am and 11:30am, the doors to rooms #26, kitchen storage and respiratory therapy storage didn't have a self-closing devices, rooms are currently being used for storage. With rooms being used as storage doors are required to have a self-closing device to maintain rating of the room.

Based on visual observation the facility failed to provide maintenance to the exit signs. This would allow for the exits to possibly not be seen or used in the case of an emergency. This would affect 1 of 6 corridor exits and potentially affect about 10 residents of the facility.
Findings:
During facility tour on 05/07/2010 between the hours of 8:30am and 11:30am, the exit sign at the end of the main medical corridor by room #30 was not illuminated at this time.
