Based on visual observation and testing there were 2 of 6 smoke barrier doors that when the Fire Alarm was Activated the doors did not close to be smoke 20 minute Fire protection rated. This deficiency could possibly affect 9 Patients in a Emergency.
Findings:
During tour on 8-1-11 between 9:30 AM and 2:00 PM it was observed that 1) Wwst barrier door in building B by nursing station and building E barrier doors needs to close and latch when Fire Alarm is Activated.

Based on visual observation and testing the facility 11 of 16 storage room doors that need to have self closures on their doors so doors will close and latch to resist the passage of smoke. This deficiency could possibly affect 9 patients in a Emergency.
Findings:
during Tour on 8-1-11between 9:30 AM and 2 00 PM it was obeserved that doors S-209 , S-207, S-205, S-211, L-125, L-122, L-128, L-129, and L-126 need self closures to resist the passage of smoke.

Based on visual observation and testing the facility's Main Electrical Room needs hard level surface from exit to safe path of travel away from building. This deficiency could possibly affect 2 persons in a emergency.
Findings:
during tour on 8-1-11 between 9:30 AM and 2:00 PM it was observed that when you exit the Main Electrical Room you step on a iron platform to exit than there is a hole about 3 Ft under the platform
without any level surface to walk on causing the exit not to be readily accessible at all times.

Based on visual observation and testins with the head of Maintanence the facility does not have Fire Alarm Protection in North Wing future Patient Rooms. This deficiency could possibly affect 9 Patients

During tour 0n 8-1-11 between 9:30 AM and 2 30: PM it was observed that there is no Fire Alarm Coverage in the North Wing Patient area.
