Based on a review of facility documents and interviews with staff (EMP) it was determined the facility failed to provide to the patient a written notification of the facility's grievance decision for two of three patient grievances reviewed.
Findings include:
A review on January 21, 2010, of the administrative policy entitled, "Formal process for Filing Grievance," revealed, "...The patient/family member will be informed in writing of the review [sic] results and final decision within 3 days of the action...."
A review on January 22, 2010, at 10:00 AM of two grievance investigations conducted by the facility revealed that a written notice had not been provided to the complainants.
An interview conducted on January 22, 2010, at 10:00 AM with EMP1 and EMP2 confirmed that written notice of the facility's grievance decision, the hospital contact person, the steps taken on behalf of the patient to investigate the grievance, the results of the grievance process, and the date of completion was not provided to the complainants.
