The facility failed to ensure that the visitation policy was followed as evidenced by lack of documentation in the visitors log. 10 out of 20 visitation sign ins lacked date, time, or name of person visiting.
Interview with CEO on 12/19/2018 at 10:30 revealed that the patient did infact have a male visitor, but it was not documented in the log or in the patient's medical chart.
