Based on observation on 9/13/2012, with the DON, the following three designated exit doors had key operated locks (installed above the panic-type fire exit hardware) that could not be unlocked from inside the building without the use of a key: exit door from main waiting room; exit door near main nurse station; exit door from staff service door at rear of building. NFPA 101, Life Safety Code requires designated exit doors be able to be unlocked from inside the building by an obvious one-action operation without the use of a key.
