Based on review of records, written treatment plans in 2 (Patients #2 and #4) of 4 patients (Patients #2, #3, #4, and #5) did not have long-range and/or short term goals identified.


Patient #2 was admitted on 5-29-16. Review of chart showed that one out of the four treatment plans initiated did not contain a long-term goal. The Potential for Self Harm (sic)/Suicidal Treatment Plan was not filled out at the top. Only the Short Term Goals section was addressed on the form.


Patient #4 was admitted on 6-15-16. Review of chart showed that a Master Treatment Plan was not in the chart. No individual treatment plan for identified problems were found. Therefore, no long-range goals or short-term goals were identified in the chart.
