Based on observation, interview, and record review, the facility failed to provide a safe environment for patients as evidenced by several deficient practices related to infection control identified during hospital tour.

Findings include:
On 08-01-13 between 9:00 and 10 a.m. during initial tour of the facility, the following infection control issues were observed:
Treatment Room (upstairs):
a. Bathroom inside the treatment room: sink dirty; toilet extensively stained with dark brown /yellow stains; shower stall had stains and dirt covering the floor. (same issues identified during previous visit).
b. Top of the crash cart was dirty and dusty.
c. Oxygen nasal cannula connected to oxygen tubing; it was uncovered and hanging over side of cart. Interview at time of observation with Interim Chief Clinical Officer (CCO/ ID #1), she stated the oxygen cannula was dirty and should have been discarded.
Equipment/Supply Room (upstairs):
a. Two (2) electric IV pumps and one (1) enteral feeding pump: visible large amounts of rust on all 3 poles holding the pumps. The bottoms of the stands were dirty and dusty.
b. In addition, there was a fabric- covered chair with numerous dark brown stains in the seat of the chair.
Dead roaches seen outside several patient rooms upstairs; Interim CCO said the facility had recently been treated for pests.
Interview on 08-01-13 at 10; 15 a.m. with the Interim CCO (Staff ID # 1) she stated she was unsure how often the facility was cleaned. She said it varied because at present there were no patients.
Review of facility policy titled " Plan: Infection Control, " dated 08-01-2012, read: " Pristine Healthcare will develop and maintains a program of surveillance, prevention, and control of infections...b. the organization will employ a coordinated process or reduce the risks of ...infection ...E. 2 The assurance of a safe hospital environment which provided quality care and necessary recourses to prevent infections ...is the responsibility of the Board of Trustees thought the hospital administrative team... "
