Based on observation, interview and record review, the facility's staff failed to follow manufacturers ' specification for maintenance of hemodialysis machines, used to provide hemodialysis treatment on patients in 2 of 5 hemodialysis machines observed in operation
Findings:
Review on 07/29/2014 of the Manufacturers Preventative Maintenance Procedure Manual # P/N 507297 page 23 directs users as follows:
" Perform the following Preventative Maintenance procedures every 12 months or 4000 hours of machine operation, whichever comes first. Perform the procedures in order of the order given below to complete the annual Preventative Maintenance and the Quarterly Preventative Maintenance. "
Observation on 07/29/2014 at 11:35 a.m. of the facility ' s in- center chronic hemodialysis unit revealed five functional Fresenius K hemodialysis machines were observed on the unit.
Review of the facility ' s preventative maintenance log revealed Fresenius hemodialysis machine # 7KOS -108540 was last provided with annual preventative maintenance on 04/02/2013.
Review of facility ' s Preventative Maintenance Log revealed Fresenius hemodialysis machine # 7 Kos -108 579 was last provided with annual preventative maintenance on 02/05/2013
During an interview on 07/29/2014 at 11:40 a.m. with Biomedical staffs (DB) who is responsible for maintaining the hemodialysis machines, revealed the annual preventative maintenance of the hemodialysis machines was not done.
