Based on document review and interview the facility failed to insure that two (2) physicians participated in the Utilization Review (UR) committee meeting based on one committee meeting held.
On 12/6/2011 at 9:00 AM in the office of the UR nurse a request to review the signature sheet for the most recent UR committee meeting revealed only one physician attended, the Chief of the Medical Staff, the UR committee meeting. The UR nurse confirmed that only one physician attended the meeting.
The facility held a UR committee meeting, but had only one physician present to carry out the function of the utilization review.
