K28 Smoke Barrier Doors (min. clear width)
Based on visual observation the facility failed to assure that installed smoke barrier doors provide approved vision panels. Doors installed for a particular purpose offers a barrier that restricts the movement of smoke from one compartment to another. The deficient practice has the potential to affect of residents.

Findings:
During the facility tour, between the hours of 9:00 AM and 3:00 PM on 7-29-25 it was observed that there are no vision panels on smoke partition doors. The Facility is requesting a waiver.

K67 Corridors Used For Return Air
Based on visual observation the facility failed to assure that the heating, ventilation and air conditioning system was installed in accordance with NFPA 90A. The system could re-circulate smoke originating from one part of the building into other parts of the building otherwise unaffected. The deficient practice had the potential to affect 10 of 10 residents.

Findings:
During the facility tour, between the hours of 9:00 AM and 3:00 PM on 7-29-15 it was observed that the Facility is using corridors for return air plenum. Facility is requesting a waiver.
