EVALUATION INCLUDES INVENTORY OF ASSETS

Based on record review and interview, the facility failed to provide an inventory of assets for 8 of 8 active sample patients (A1, A4, A5, A7, A10, A14, A16, A17). This failure resulted in the physician's inability to guide the treatment team in developing a plan of care that builds on the patient's assets/strengths.
Findings are:
A. Record Review
The following psychiatric evaluations were reviewed and found not to include patient's assets/strengths (Dates of psychiatric evaluations are in parenthesis):
A1 (3/30/12); A4 (3/30/12); A5 (3/30/12); A7 (3/27/12); A10 (3/28/12); A14 (4/2/12); A16 (4/2/12) and A17 (4/2/12)
B. Staff Interviews
In an interview with the Medical Director on 4/4/12 at 9:45a.m., the Medical Director agreed with the surveyor that the psychiatric evaluations of all the 8 active sample patients did not include patients' strengths/assets.

Based on record review and staff interview the Medical Director failed to ensure that psychiatric evaluations for 8 of 8 active sample patients (A1, A4, A5, A7, A10, A14, A16, A17) included inventory of assets/strengths.
