PATIENT RIGHTS: CARE IN SAFE SETTING

Based on observation and interview, the facility failed to ensure all medical equipment used for patient care was properly cleaned and disinfected between each patient use. This finding has the potential to cause harm to all patients receiving care in this facility.

Findings include:
While touring the facility on 10/23/2013 at 9:40 am, the following observations were made:
1. Bariatric Transfer device sitting in main hall storage area appeared dusty and dirty at the base;
2. IVAC machines x 4 sitting in the hallway against the wall appeared dirty and dusty;
3. IV stands with glucometers attached x3 sitting in the hallway against the wall appeared dirty and dusty;
4. Detector Scale x1 sitting at the end of the hallway appeared dirty and dusty.

An interview with staff #5 on 10/23/2013 at 10:00 am confirmed the findings.

The facility failed to require entries on the Code 44 Record to be complete with times and necessary information. 6 of 10 (#3, 4, 5, 6, 8, and 9) Code 44 records lacked documentation of times and or patient disposition.

Findings include:

Review of patient records revealed a facility form titled, "Code 44 Record," where documentation related to a patient's treatment for cardiac and/or respiratory arrest is entered.

The following entries were missing on patients' Code 44 Records:
-Patient #3- MD arrival time, code stop time, and patient disposition;
-Patient #4- MD arrival time, and code start and stop times;
-Patient #5- MD arrival time and CRNA (Certified Registered Nurse Anesthetist) arrival time;
-Patient #6- patient disposition;
-Patient #8- MD arrival time, CRNA arrival time, and code stop time;
-Patient #9- MD arrival time.

During an interview on 10/23/13 at 12:45pm, staff #3 confirmed these findings.
