Based on observation and interview the facility failed to separate clean and dirty in the dialysis store room. These finding have the potential to cause cross contamination of clean equipment and supplies which could increase the risk of infection to the patient by allowing clean equipment to come in contact with dirty equipment and supplies.

Observation in the dialysis set up area on 07/20/2011 the storage room across the hall from administration was observed to have clean and sterile items sitting on the supply cart with shipping boxes, bleach bottles and empty plastic containers.

Interview with staff # 02 on 07/19/2011 at approximately 1130 a.m., confirmed the clean and dirty were sitting on the same supply cart.
