Based on interview and record review, the facility failed to ensure a registered nurse (RN) supervised and evaluated the nursing care for each patient, in that, 5 of 10 (Patient #4, #7, #8, #9, and #10) patient's records did not evidence a daily registered nurse assessment.

Findings Included:

Patient #4's record did not evidence a registered nurse assessment for 7/5/16.

Patient #7's record did not evidence a registered nurse assessment for 7/8/16

Patient #8's record did not evidence a registered nurse assessment for 8/11/16

Patient #9's record did not evidence a registered nurse assessment for 1/13/17 and 1/23/17.

Patient #10's record did not evidence a registered nurse assessment for 1/15/17 and 1/21/17

During an interview on 1/24/17 at 1:35 PM, Personnel #1 confirmed there was not any RN assessments done on Patient's #4, #7, #8, #9, and #10. There were only LVN (licensed vocational nurse) assessments completed for those days.

Based on interview and record, the hospital failed to complete a discharge summary within 30 days of the patient's discharge, in that 1 of 7 (Patient #6) patient's discharge summary was not completed within 30 days of discharge.

Findings Included:

Patient #6's medical record reflected the patient discharge date of 7/27/16. The discharge summary reflected a dictation date of 9/5/16.

During an interview on 1/24/17 at 12:40 PM, Personnel #1 verified the discharge summary was completed more than 30 days after discharge.
