Based on document review and staff interview, the Long Term Acute Care Hospital (LTACH) administrative staff failed to report 1 of 1 incidents of possible dependent adult abuse of Patient #1 by Employee A to the Iowa Department of Inspections and Appeals. Failure to report incidents of possible dependent adult abuse may result in administrative staff allowing staff members to continue to work with patients after members committed possible acts of abuse, and/or delay in the investigation related to allegation of dependent adult abuse. The LTACH identified an average daily census of 28 patients.
Findings include:
1. During an interview on 12/12/19 at 11:58 AM CEO revealed LTACH had first learned of allegations of dependent adult abuse when a detective came to the hospital on 12/06/19. CEO further stated they began the internal investigation on 12/06/19.
2. Review of LTACH's internal investigation into possible dependent adult abuse of Patient #1 revealed in part:
12/06/19 -- CEO met with Detective and learned of the nature of this allegation. LTACH began their internal investigation.
3. Review of policy, "Abuse, Neglect," last revised 07/01/18 revealed in part, "All incidents will be..reported to applicable agencies..."
4. During an interview on 12/12/19 at 2:45 PM, CEO confirmed that LTACH failed to notify Iowa Department of Inspections and Appeals of the alleged dependent adult abuse.
