Based on the review of facility documents and interviews completed July 19 &20, 2011, the facility failed to provide evidence two (#8 & #10) of five (#6, #7A&B, #8, #9 & #10) medication errors were reported to the physician. The findings included:
A. The Pharmacy Profile Override documented medication error #8 occurred at 11:39 AM on July 16, 2011. In an interview at 11:22 on July 20, 2011, the Chief Nursing Officer (CNO) verified there was no evidence the physician had been notified of the medication error.
B. Medication Error #10 occurred on April 28, 2011, at 1700. The patient was given a medication that was on hold. During an interview at 11:25 AM on July 20, 2011 the CNO verified the facility had no documentation to verify the physician had been informed the medication error occurred.
