Based on medical record review, staff interviews, and policies review, the facility failed to meet patient safety standards by not following the facility Tobacco Free Campus policy.

Findings were:

The facility failed to ensure patient safety by not enforcing patient compliance with the Rehabilitation Hospital, Tobacco Free Campus policy. As per the facility video, patient#1 wheeled herself outdoors to the parking lot, and burned self with cigarette. Upper chest and chin area were burnt. The patient was transferred to UMC and was admitted for care and treatment.

The finding was acknowledged by staff #1 and staff #2 the morning of 03/31/2021.
