Based on document review and interview, the facility failed to follow physician orders in 1 of 10 patients reviewed.
Findings include;
1. The medical record (MR) indicated an order for staff to stay with patient in the bathroom on 3/9/22 at 6:03 PM and order for line of sight (LOS) was received on 3/11/22 at 6:13 AM.
2. MR lacked documentation that the LOS order was followed on 3/11/22, 3/12/22, 3/13/22, 3/14/22, 3/15/22, 3/16/22, and 3/18/22.
3. MR lacked documentation that staff followed order to stay with resident in bathroom on 3/18/22, at time of patient (P1)fall.
3. Staff member # S3 (Quality Manager) confirmed in interview at approximately 1:00 PM on 6/24/22 that the MR lacked documentation of LOS and to stay with patient in bathroom on dates indicated above.
