On 5/2/2013 at approximately 1455 hours, it was revealed by observation and interview that multiple ceiling tiles were missing in the east data closet.

Based on observations it was determined that the health care facility failed to maintain fire protection components throughout the facility. This has the possibility to affect 100% of the residents.
The Findings Include:
On 5/2/2012 at approximately 1400 hours, it was revealed by observation and interview that the nozzle placement for the kitchen appliances under the hood was not correct.
On 5/2/2013 between 1330 hours and 1515 hours, it was revealed by observation and interview that a there was dust build up on sprinkler heads throughout the facility.

Based on observations it was determined that the health care facility failed to maintain the excessive build up of dust and combustible storage. This has the possibility to affect 75% of the residents.
The Findings Include:
On 5/2/2013 at approximately 1330 hours, it was revealed by observation and interview that there were combustibles in the boiler room.
On 5/2/2013 at approximately 1500 hours, it was revealed by observation and interview that there were combustibles in the east electrical room.
On 5/2/2013 at between 1330 and 1515 hours, it was revealed by observation and interview that there was excessive dust build up on vents and returns throughout the facility.

Based on observations it was determined that the health care facility failed to properly secure a compressed gas cylinder.
The Findings Include:
On 5/2/2013 it was revealed by observation and interview that the compressed gas cylinder for the soda fountain was not properly secured. Statewide Fire Prevention Code section 3003.5.3 requires the proper securing of compressed gas cylinders from falling over.

On 5/2/2013 at approximately 1430 hours, it was revealed by observation and interview that there was an alcohol based hand rub dispenser mounted directly over a light in patient room 118.
