Based on staff interview,review of facility policy and procedure and review of facility employee personnel and health files, on 9/13/17 the day of the Federal Conditional revisit survey, it was determined that the facility failed to ensure all contracted employees undergo employee health screenings prior to commencing employment.
Findings include:
Reference: Facility Human Resources policy titled, "Employee Health", policy code: HR-503 states, "I. Employee Health Services Matheny shall provide the services of a health program to all staff, managed by an employee health nurse/coordinator and supervised by a physician. ... 1. Post-offer, pre-employment medical examinations - Prior to commencing services at Matheny, all new staff members shall undergo a medical examination that includes all aspects required by law. ... 2. Tuberculosis screenings - Unless exempt under New Jersey law, all Matheny staff members shall undergo tuberculosis screening post-employment offer, but prior to commencing employment"
1. Upon review of employee personnel and health records for Staff #4 and Staff #7, the files evidenced the following:
a. Health Records of Staff #4 and Staff #7 lacked evidence of tuberculosis screening, in accordance with CDC guidelines and the facility's policy.
b. Both health records lacked evidence of a pre-employment medical examination, in accordance with facility policy.
2. Upon interview with Staff #9 (Employee Health Coordinator), he/she stated, "I do not look over the per diem health files."
