Based on observation and staff interview, the facility did not ensure that the smoke doors between smoke compartments are intact and or constructed to resist the passage of smoke between the corridors and the area patient care units in the event of smoke or fire.

Findings include:

The smoke door in the back of the ICU (between the ICU unit and the corridor) was observed to have a gap between the two leaves of the door of approximately 1/2- 1 inch when the door is closed. This gap permits the passage of smoke in the event of fire or smoke. This finding was brought to the attention of the facility management on the afternoon of 1/9/2012.

2- The smoke door in the back of the CCU (between the CCU unit and the corridor) was observed to have a gap between the two leaves of the door of approximately 3/4 inch when the door is closed. This gap permits the passage of smoke in the event of fire or smoke. This finding was brought to the attention of the facility management on the afternoon of 1/9/2012.
