Based on observation and staff interview the facility failed to ensure self closing doors of hazardous areas closed to a latching position. This has the potential to affect all patients receiving services at the facility. The facility census was 17.
Findings include:
Observation of the soiled utility room in the West tower completed on 03/14/16 revealed the door was on a self closer but when tested the door failed to close to a latching position. This finding was verified by Staff C at the time of the observation.

Based on observation, documentation review, and staff interview the facility failed to ensure all components of the fire alarm system functioned and failed to ensure the fire alarm system was functioning from 12/16/14 through 12/07/16. This has the potential to affect all patients at the facility. The facility census was 17.
Findings include:
1. Review of the Work order form issued on 12/16/14 completed on 03/14/16 revealed the Honeywell Fire Alarm System was not responding, all fuses good rebooted and still no response call security to put fire watch in place for Ross Park Building (West tower) and called Honeywell for service. Quotes were received from Honeywell and Simplex for quote for a new fire panel and administration gave permission to go with simplex.
2. Review of the Fire Alarm Inspection Report dated 03/19/15 completed on 03/14/16 revealed West tower fire alarm devices were unable to be tested during the inspection due to being Honeywell equipment. This was confirmed by Staff A at the time of the review.
3. Review of the Initiating and supervisory deice tests and inspections dated 12/07/15 revealed all devices in the West tower revealed all devices passed the initial inspection after install.
4. Observation of the manual pull station located in the administrative corridor completed on 03/15/16 revealed a sign stating pull station and fire system out of order in case of emergency call 740-283-7911. This observation was confirmed by Staff A at the time of the observation.
Interview with Staff A completed on 03/15/16 at 9:00 AM revealed that the pull station and alarm system works but just waiting on city inspection before letting it be used. Documentation provided revealed pull stations on all 4 floors were tested and signal was received at the alarm panel on 03/02/16.
