Based on record review and interview, the facility failed to ensure that Master Treatment Plans MTPs) identified patients' personal strengths, abilities, or assets which can be utilized for treatment in 4 of 4 active records reviewed (A1, A2, A3, and A4). The failure to include patients' personal strengths, abilities, or assets into the master treatment plan may result in prolonged hospitalization by making the master treatment plan less individualized and less specific for each patient. Findings include:
A. Record Review
1. The treatment plan for Patient A1, dated 8/16/11 did not include strengths, abilities, or assets.
2. The treatment plan for Patient A2, dated 8/16/11 did not include strengths, abilities, or assets.
3. The treatment plan for Patient A3, dated 8/17/11, did not include strengths, abilities, or assets.
4. The treatment plan for Patient A4, dated 8/16/11, did not include strengths, abilities, or assets.
B. Interviews
1. In an interview on 8/22/11 at 10:20a.m., treatment plans were discussed with the Director of Nursing. She agreed that: "No, there are no strengths listed on the treatment plans."
2. In an interview on 8/23/11 at approximately 10a.m. the Medical Director agreed with the findings.

Based on record review and interview, the Medical Director failed to adequately ensure the quality and appropriateness of services provided by the medical staff. Specifically, the Medical Director failed to ensure that Master Treatment Plans (MTPs) utilized information on patient strengths, abilities, or assets for 4 of 4 active sample patients (A1, A2, A3 and A4). Failure to incorporate patient strengths, abilities or assets in the master treatment plan diminishes the effectiveness of treatment and may result in prolonged hospitalization. (Refer to B119)
