Based on observation, the facility failed to maintain all fire doors.
The findings include:
Observation on 6/2/16 at 11:00 AM revealed the 90 minute cross corridor fire doors that lead into the Patricia Neal Unit, the lower latch does not extend 5/8 of an inch into the floor strike per the label on the fire doors.
This finding was verified by the engineering team and acknowledged by the CEO during the exit conference on 6/2/16.
