Based on observation and interview, it was determined that the Surgery Waiting Room, which was open to the corridor and not under the direct supervision of a nurses station, did not have a smoke detector as required by the National Fire Protection Association, NFPA 101, 2000 Edition, 19.3.6.1, Exception 1 or 2. As a result an immediate response to fire or smoke to protect the corridor could not be assured. The failed practice had the potential to affect all patients, staff and visitors by being unable directly supervise space open to an exit access corridor. The findings were:

On a tour of the facility with the Director of Plant Operations and the Safety Officer on 06/7/11 at 1400 revealed the Surgery Waiting Room did not have a smoke detector. The rooms were found to be open to the corridor and were not under the direct supervision from a nurse's station or similar space. By interview this was confirmed with the both the Director of Plant Operations and the Safety Officer at the same time.

Based on observation during a tour of surgical anesthetizing locations, Operating Rooms 1 and 2, and the Cesarean Section Room, it was determined that a Extension Cords and Multiple Outlet Plug Strip Devices were used. The failed practice had the potential to affect all surgical patients due to the possibility of malfunction of Extension Cords and/or Multiple Outlet Plug Strip Devices. The findings were:

A. Observation and interview with the Director of Nursing on 06/7/11 at 1500 revealed the use of Extension Cords and Multiple Outlet Plug Strip Devices. This was confirmed with the at the same time with the Director of Plant Operations and the Safety Officer.
B. National Fire Protection Association, NFPA 99, 1999 Edition, Section 7-5.1.2.5 Line Voltage Equipment - Anesthetizing Locations. Flexible cords used in anesthetizing locations shall be continuous and without switches from the appliance to the attachment plug, where the attachment plug can be inserted only in a fixed approved receptacle. The use of Extension Cords and Multiple Outlet Plug Strip Devices, are prohibited for use in anesthetizing locations, unless provided for by Code Exception.
