Based on facility observation and staff interview and verification, the facility failed to ensure that emergency lighting of at least 1½ hour duration was provided in accordance with requirements referenced in NFPA 101, 7.9, NFPA 101, 18.2.9.1 and NFPA 110, 5.3. The facility had a capacity of 93 beds with a census of 70 patients.

Findings included;

On 05/21/13 between 8:00 A.M. and 11:50 A.M. tour of the facility was completed with Staff Q and R. Observation of the facility emergency power source revealed the presence of two emergency generators. Each generator was located in a large enclosed area. Observation of the location for the generator identified as the 710 revealed there was no emergency lighting in the area.

Both staff present on the tour verified there was no emergency battery operated lighting in the enclosed area for generator 710.

Based on facility observation and staff interview and verification, the facility failed to ensure the required automatic sprinkler system was continuously maintained in reliable operating condition. The facility had a capacity of 93 beds with a census of 70 patients.

Findings included;

On 05/21/13 between 8:00 A.M. and 11:50 A.M. tour of the facility was completed with Staff Q and R. The following observations were made with regards to maintenance of automatic sprinkler heads for proper functioning;

1. Observations of a materials storage room revealed the presence of shelving in the center of the room. Located on the shelves were boxes of various supplies. At least five boxes were stored on the top shelves which were then within 18 inches of automatic sprinkler heads. Staff Q verified the boxes were too close to the sprinkler heads. Staff Q asked other staff working in the area to immediately remove the boxes from the top shelves and informed them why.

2. Observation of the kitchen and dietary supply storage areas revealed at least 5 sprinkler heads with missing escutcheon rings. One sprinkler head was recessed within the ceiling tile. Staff Q and R present on tour verified the findings and implemented corrective measures immediately.
