Based on record review and interview with hospital staff, the hospital does not ensure pharmaceutical services/the Consultant Pharmacist in consultation with hospital staff monitors the discrepancies from the automatic drug-dispensing machines to evaluate the potential for medication errors and drug diversion.
Findings:
1. Staff D stated on 01/23/14 in the morning that all discrepancies from the Omnicell (automatic drug-dispensing machines) are investigated by nursing and they (drug room) do not get any feedback on the discrepancies or participate in the analysis of the discrepancies.
2. Review of Pharmacy and Therapeutics meeting minutes provided for review did not have any evidence that Omnicell discrepancies were analyzed by the pharmaceutical services of the hospital.
