Based on review of paramedic personnel files, interviews, and review of the Rules and Regulations for Emergency Medical Services, January 23, 2017 and interview, it was determined the medical staff and governing body failed to approve and grant privileges to three of three (#1-3) paramedics reviewed. The failed practice did not assure Paramedics #1-#3 had oversight to assure they were practicing within the scope of their privileges and license. The failed practice affected Paramedics #1-3. Findings follow:
A. Review of the personnel files for Paramedics #1-3 showed no privileging or credentialing documents.
B. The Executive Assistant stated during an interview at 3:30 PM on 03/07/18 the facility did not credential or privilege the paramedics.
C. The Chief Nursing Officer stated during an interview at 2:20 PM on 03/08/18 the paramedics were approved as a group and the job description was the privileges for the paramedics.
D. Review of the Rules and Regulations for Emergency Medical Services, January 23, 2017, showed:
1) The facility's medical staff must approve the privileges granted to the individual EMSP with the agreement of the hospital's governing body.
2) Specific policies governing the supervision of the EMSPs were to be developed.
3) The procedures to be performed by the EMSP must be developed by the hospital medical staff and also approved by the governing body.
