Based on record reviews and interview, the hospital and DON failed to ensure non-employee licensed nurses who were working in the hospital were provided adequate supervision by an appropriately qualified hospital-employed RN as evidenced by having a contracted agency nurse (SF15ARN) working as the charge RN with no hospital-employed RN present in the hospital to provide supervision during the night shifts (5:00 p.m. to 5:00 a.m.) of 01/13/18, 01/20/18, and 01/27/18.
Findings:
Review of the list of contracted agency nurses, presented by SF1ADM, who provided nursing services from 01/02/18 through 02/05/18 revealed SF15ARN was listed as a contract agency RN.
Review of the nursing staffing schedules from 12/31/17 through 02/06/18, presented by SF1ADM, revealed SF15ARN worked as the charge RN on the night shifts of 01/13/18, 01/20/18, and 01/27/18. Further review revealed no documented evidence that a hospital-employed RN was present during these times to provide supervision to the contract agency RN.
In an interview on 02/07/18 at 9:40 a.m. with SF1ADM, SF3QC, and SF7VPC present, SF1ADM confirmed that SF15ARN was the charge RN on the above-listed night shifts. She indicated a hospital-employed Licensed Practical Nurse was present during each shift. SF1ADM confirmed a hospital-employed RN was not present to provide supervision of SF15ARN during the night shifts of 01/13/18, 01/20/18, and 01/27/18.
