Based on visual observation the facility failed to assure that one hour fire rated construction encloses the vertical openings between floors. This reduces or eliminates the potential of fire and/or smoke spreading from one floor to another. This deficiency has the potential to affect 7 of 7 patients.
Findings:
During the facility tour, between the hours of 9:00 am to 2:15 pm on 12/4/2017 and 10:30 am to 12:30 pm on 12/5/2017 it was observed the three story elevator shaft walls for number one and two elevators have several shaft wall penetrations, are lacking horizontal steel framing tracks within shaft wall and have exposed structural I beams within the shaft wall.
Interview with Facilities Support Supervisor revealed the facility was not aware that the fire rated construction of the vertical elevator shaft was compromised.

Based on visual observation and testing the facility failed to provide a Type 1 Essential Electrical System powered by the emergency generator. The Type 1 system requires the electrical system to include an automatic transfer switch to transfer/retransfer the load automatically in order to illuminate an interruption in power. The deficiency has the potential to affect 7 of 7 residents.
Findings:
During the facility tour and testing of the systems, between the hours of 9:00 am to 2:15 pm on 12/4/2017 and 10:30 am to 12:30 pm on 12/5/2017 it was observed the 400 KW generator is lacking a manual remote stop on the outside of the 400 KW generator's weather proof enclosure.
Interview with Facilities Support Supervisor revealed the facility was not aware that the emergency generator did not have a manual remote stop located on the outside of the generator weather proof enclosure.
