Medical record review of Patient ID #3 record with Director of Nursing Staff ID # 54, included the following entries for hygiene (bathing/shower) for hospital stay 08/20/21 - 09/07/21:

Between the dates of 8/20/2022 and 9/7/2022, there was 5 documented baths (which included bathing wipes) in 16 days per the medical record of Patient ID #3.
Interview with DON Staff ID #54 on 3/6/23 at 2:45 p.m., he confirmed the facility policy stated bathing or showering would be provided every day and as needed. He confirmed the medical record for Patient ID #3 failed to demonstrate that this occurred.

Record review of facility policy "Patient Bathing" dated 3/1/2020 showed the following information:
I. POLICY AND GENERAL STATEMENT
Patient bathing practice is an important strategy to reduce the risk of hospital acquired infections. Appropriate bathing practices enhance removal of transient soil and microorganisms and reduction of resident microbial count on patient skin. Bathing for inpatients should occur at least daily and as needed, to maintain clear skin.
Medical record review on 3/6/23 for patient ID #9 with Infection preventionist ID # 59 and regulatory Resource Nurse ID# 63 showed missing entries for hygiene (bathing/shower) for current hospital stay on the following dates: 2/7/2023, 2/23/2023, 2/24/2023 and 2/27/2023.
Interview with both staff ID # 59 and ID #63 on 3/6/2023 at 1215 PM stated that patients are to receive hygiene (bath/shower) daily and it should be documented in the patient's medical record.
