Based on observation and interview, the facility failed to maintain a corridor per the requirements of:
2012 NFPA 101, 19.2.3.4 (5)
This deficiency could affect 10 residents.
Findings include:
On 6/21/2018, during a tour of the facility from 2:00 pm to 3:30 pm, the surveyor observed unsecured furniture in the means of egress corridor located at Pharmacy Room 116.
A member of the maintenance staff was present when this deficiency was identified.

Alcohol Based Hand Rub Dispenser (ABHR)

Based on observation and interview, the facility failed to maintain the Alcohol Based Hand Rub (ABHR) dispensers per the requirements of:
2012 NFPA 101, 19.3.2.6 (11) (f)
Findings include:
On 6/20/2018, during a tour of the facility from 8:00 am to 11:30 am, the facility failed to provide documentation of the testing of ABHR dispensers per the manufacturer's care and use instructions each time a new refill was installed.
A member of the maintenance staff was present when this deficiency was identified.

Based on observation and interview, the facility failed to maintain the designated smoking area per the requirements of:
2012 NFPA 101, 19.7.4* (6)
Findings include:
On 06/20/2018, during a tour of the facility from 8:00 am to 11:30 am, the surveyor observed the designated smoking area's metal container with a self-closing cover device for ashtrays to be emptied into being used as a garbage can, cigarette packs and paper items were observed in the can.
A member of the maintenance staff was present when this deficiency was identified.
