Based on observation, it was determined that, the facility failed to ensure the sprinkler system was continuously maintained.

Findings include:

MORRIS COUNTY FACILITY

1. On 8/16/11 at 11:10 AM, in the presence of Staff #1, in the medical records room, 18 charts were stored within 18" of the sprinkler, creating an impedance to the sprinkler.


WAYNE FACILITY

1. On 8/25/11 at 10:50 AM, in the presence of Staff #1, in the storage closet, items were stored within 18" of the sprinkler, creating an impedance to the sprinkler.

Based on observation, it was determined that, the facility failed to ensure Alcohol Based Hand Rub dispensers (ABHR) were not installed above ignition sources.

Findings include:

1. On 8/16/11 at 11:05 AM, in the presence of Staff #1, in the soiled utility room, an ABHR was installed above an electrical outlet.
