Based on observation, tour, and staff interview, the facility failed to maintain the proper replacement sprinkler head types used in the facility in the spare sprinkler head box. This had a potential to affect all patients, staff and visitors to the facility. The census at the time of survey was 23.

Findings include:

During the tour of the facility with Staff A on 02/23/15 at 10:18 AM the spare sprinkler head box, located in the equipment room in the basement, was inspected and failed to have all types of sprinkler heads in the box. The replacement sprinkler heads used in the emergency stairways, the trash, and laundry chutes were not present at the time of inspection. This finding was confirmed on 02/23/15 at 10:18 AM by Staff A.

Based on observation, tour and staff interview the facility failed to ensure the door to the trash chute closed properly. This had a potential to affect all patients, staff, and visitors to the facility. The census at the time of the survey was 23.

Findings Include:

During the tour with Staff A on 02/23/15, at 10:15 AM, the trash chute located in room 6013 was equipped with an automatic door closing device. The closing device failed to close and latch the trash chute door after use. The finding was confirmed on 02/23/15 at 10:15 AM by Staff A and Staff B.

Based on observation, tour, and staff interview, the facility failed to maintain the exit access door to the public way. This had a potential to affect all patients, staff, and visitors to the facility. The census at the time of the survey was 23.

Findings include:

1. During the tour with Staff A on 02/23/15, at 11:05 AM, the exit access at the grade level of stairway C failed to open without excessive force. The door stuck at the upper right corner and significant force was required to open the door. This finding was confirmed by Staff A at 11:05 AM.

2. During the tour with Staff A on 02/23/15 at 10:30 AM the walkway outside the exit access from Stairway A was covered in snow and ice. The finding was confirmed by Staff A and Staff B on 02/23/15 at 10:30 AM.
