Based on observation the facility failed to provide corridor doors with no impediment to closing to 2 of 43 corridor doors
Findings:
Observation on 12/28/2010 between 11:00 AM and 2:30 PM revealed the corridor doors to 400 hall clean linen and case management are rubbing against the frame causing an impediment to closing.
This deficiency may adversely affect 6 residents and 5 staff

Findings:
Observation on 12/28/2010 between 10:00 AM and 2:30 PM revealed one electrical outlet in room 501 has a broken top socket and may cause an electrical shock if used.
