Based on review of medical records and hospital documents and interviews with hospital staff, the hospital failed to ensure the patient's representative was kept informed by the physician of the patient's health status, plan of care and treatment. This occurred in one of four medical records (Record #1) reviewed.

Findings:

1. The hospital's patient rights include the right of the patient to have a representative involved in their care planning.

2. Although both nursing and case management disciplines noted Patient #1 ' s representative requested multiple times to talk with the physician about the patient, the documentation reflected only one time nursing had reached the physician and the patient ' s representative was able to talk with the physician.

3. This finding was reviewed and verified with hospital staff in the afternoon of 10/31/2011.
