K29 Hazardous Areas
Based on visual observation the facility failed to maintain the separation of hazardous areas from other parts of the building, including the egress corridor. Hazardous areas are required to be constructed to resist the passage of smoke. The deficient practice had the potential to affect 23 of 23 residents.
2 of 3 smoke compartments have hazardous areas that are not separated.
Findings:
During the facility tour, between the hours of 9:30 AM and 2:30 PM on 7-27-16it was observed that , 1) Solied Linen 2 door latch was tapped over latch causing the door not to close and latch in its frame. 2 ) Warehouse
Storage on first floor was x-ray room and now is storage this needs to be sealed to resist the passage of smoke.
Interview with Maintenance Supervisor revealed the facility was not aware of the walls of the hazardous areas having unsealed penetrations.

K34 Stairs/Smokeproof Towers
Based on visual observation the facility failed to assure that one hour fire rated construction encloses the openings between floors. This reduces or eliminates the potential of fire and/or smoke spreading from one floor to another. This deficiency has the potential to affect 23 of 23 residents.
Findings:
During the facility tour, between the hours of 9:30 AM and 2:30 PM on 7-27-16 it was observed that the facility has 8 32 gallon containers inside stairwell 1. This has the potential to cause fire or smoke spreading from one floor to another
Interview with Maintenance Supervisor revealed the facility was not aware that the fire rated construction of the stairwell was compromised.
