Based on observation, and interview, it was determined three of three emergency generators serving the facility were supplied with maintenance-free batteries. Battery cells cannot be inspected on maintenance-free batteries, which assure batteries are capable of starting the generator if normal power is lost which has the potential to affect the health and safety of patients. Maintenance-free batteries are not allowed on Type 1 Essential Electrical Systems per NFPA 110, Section 3-5.4.5. The failed practice had the potential to affect all patients, staff, and visitors. The facility had 39 patients on 08/23/11. The findings follow:
A. Observation of the emergency generators on 08/25/11 at 1300 with the Director of Engineering revealed the three generators serving the facility were supplied with two maintenance-free batteries each.
B. Iin an interview conducted on 08/25/11 at 1310, the Director of Engineering verified all generators were supplied with maintenance free batteries.

Based on review of the Preventative Maintenance Manual (2011) and interview, it was determined the facility failed to perform monthly tests for four of four Line Isolation Monitors 2011 in accordance with NFPA 99 Section 3-3.3.4.2 (b). The failed practice had the potential to affect all patients in critical care areas. The facility had nine surgical patients on 08/23/11. Failure to test the line isolation monitors had the potential to affect the health and safety of surgical patients because the proper function of the monitors to prevent electric shock was not evaluated. The findings follow:
A. Review of the Preventative Maintenance Manual for 2011 on 08/24/11 at 1005 revealed "Preventative Maintenance Procedure #51" required annual testing of the Line Isolation Monitors located in the facility. The most recent date documented for testing of the devices was 06/02/09.
B. In an interview conducted on 08/24/11 at 1100, the Director of Engineering verified there was no further Line Isolation Monitor testing documentation available for review.
