Based on observation, resident and family interview, staff interview, and review of facility policies, the facility failed to provide a clean and sanitary environment for four patients (Patients #6, #7, #8, and #9) with the potential to affect all patients receiving services from the facility.
Findings include:
Interview on 09/17/24 at 10:23 A.M. with Patient #6 and her parents revealed the floor and the intravenous (IV) pole were filthy. Observation of the IV pole and the floor next to it revealed dried brown liquids withv multiple medical equipment packaging waste on the floor. Patient #6's mother reported the brown stuff had been there greater than a week.
Interview on 09/17/24 at 10:25 A.M. with Staff A confirmed the waste on the floor and the dried brown liquids on the IV pole and floor in Patient #6's room.
Observation of the medical equipment being used for Patient #7 on 09/17/24 at 10:30 A.M. revealed multiple stains of dried brown liquid on her floor and all over the bottom of her IV pole. Interview at the time of the observation with Staff A confirmed the stains.
Observation on 09/17/24 at 10:32 A.M. of the common area in the room shared by Patient #8 and Patient #9 revealed dried brown liquid stains in front of the garbage can. There were multiple medical equipment wrappers strewn on the floor. Interview during the observation with Staff A confirmed the observation.
Interview on 09/18/24 at 09:08 A.M. with Environmental Services Staff G confirmed floors are to be mopped in patient rooms at least daily, if not more. He confirmed the environmental services staff, along with the clinical staff, are responsible for cleaning the bottom of the IV poles. He reported he did not know why some rooms were found with substances dried on the floor. He also confirmed there were enought staff and cleaning supplies to complete their job timely.
Telephone interview on 09/18/24 at 9:21 A.M. with Registered Nurse (RN) Staff I confirmed clinical staff as well as housekeeping are to clean up any spills or trash on the floor if they put it there.
Review of the facility policy titled "Routine Daily Cleaning and Disinfection," revised April 2023, revealed the clinical staff will be responsible for daily disinfecting of equipment such as medication carts and wound care supply carts. All patient rooms, clinical support, and ancillary areas will be cleaned daily. Wet mopping of the floors and baseboards are to be completed daily.
Review of the facility policy titled "Cleaning: Environment, Patient Equipment and Medical Devices," revised October 2023, revealed the standard of cleanliness was all parts of the equipment, including the underneath, will be visibly clean with no blood and body substances, dirt, debris, dust, adhesive tape, stains or spillage.
