Based on document review and interview the facility failed to ensure nursing staff developed interventions for the goals identified for 11 of 11 medical records reviewed (#2, #4 - #13). Findings include:
On 6/30/14 from approximately 1300 to 1600 during medical record review it was revealed that medical records #2, #4 - #13 failed to include nursing interventions for identified patient goals on the nursing care plan.
On 6/30/14 from approximately 1300 to 1600 during interview with staff A it was confirmed that medical records #2, #4 - #13 failed to include interventions. Staff A stated "I guess until you pointed it out I didn't notice that the care plans didn't have interventions."
