1. Based on observation, three of three Exit Signs in the corridors of the patient wing not illuminated. The failed practice had the potential to affect all patients admitted to the facility. The findings follow:
A. Observation of the corridors on the patient wing on 07/06/10 from 1400 to 1500 with the Director of Maintenance revealed three Exit Signs were not illuminated.
B. The Director of Maintenance confirmed the findings on 07/06/10 from 1400-1500.

2. Based on observation, it was determined Alcohol Based Hand Rub (ABHR) dispensers were located over electrical receptacles in patient rooms. The failed practice had the potential to affect all patients admitted to the facility. The findings follow:
A. Observation on 07/06/10 at 1300-1400 with the Director of Maintenance revealed Patient Rooms 101, 103, 105, 108 and 119 had ABHR's located over electrical receptacles.
B. The Director of Maintenance confirmed the findings on 07/06/10 at 1300-1400.
