This standard is not met as evidenced by:
Based on observation and testing, the facility failed to have a properly installed fire alarm system as required by NFPA 72 sections 2-8.1 and 1-5.6. These conditions affected four (4) of seven (7) smoke compartments.
Findings Include:
On December 4, 2015 at 10:30 a.m., observation revealed two (2) manual fire alarm boxes mounted beyond the 5 feet requirement from exit door in the following areas:
1. The manual pull station in Zone 4 (X-Ray Unit) was mounted 12 feet from the exit door.
2. The manual pull station in Zone 5 (Business Office) was mounted 9 feet from exit door
Findings include:
On December 4, 2015 at 11:15 a.m., it was observed that there were no emergency strobes lights and horn systems installed in the Emergency Room and Step-Down Unit of the facility.
The finding was acknowledged by the Administrator and verified by the Maintenance Supervisor during the exit interview.

Based on documentation review the facility failed to provide written records of test in accordance with NFPA 101 sections 19.3.4, 9.6. This deficient practice affected the entire facility on the day of the survey.
Findings include:
On December 4, 2015 at 12:00 p.m., the maintenance person could not produce written records and documentation for the annual inspection of the fire alarm system.
The finding was acknowledged by the Administrator and verified by the Maintenance Supervisor during the exit interview.

This standard is not met as evidenced by:
Based on observations, the facility failed to properly inspect and test sprinkler system within the past 12 months as required by NFPA 25, 9.7.5. This deficient practice affected the entire facility on the day of the survey.
Findings include:
On December 4, 2015 at 11:45 a.m., the maintenance
person could not produce the current documentation of the annual inspection of the sprinkler system within the past 12 months.
The finding was acknowledged by the Administrator and verified by the Maintenance Supervisor during the exit interview.
